How to Request a New State Appointment

Submit a new state appointment request for an existing carrier contract, including what to prepare and carrier-specific requirements.

If you're already contracted with a carrier and want to start selling in a new state, you'll need to request a new state appointment. This article walks through how to submit the request, what you'll need, and what to expect from each carrier.

Before you submit

You must be Ready to Sell with the carrier before requesting a new state. Check your RTS status under Back Office → Carrier Contracts in your Spark platform account.

You'll also need a copy of your individual state license for each state you're adding. You can download state licenses from your NIPR account at nipr.com.

📣 Aetna will not approve a state addition without a license upload. If you skip the upload, your request will be rejected and you'll need to resubmit with the license included.

Carriers that require their own portal

The following carriers do not use the Spark form or platform for state additions — you must add the state directly through the carrier's portal instead:

  • Wellcare

  • UHOne

  • SCAN

  • Alignment

  • Devoted

  • HealthSpring (Cigna)

How to submit for all other carriers

For all other carriers, you can submit two ways:

  1. Through the Spark platform — go to Back Office → Carrier Contracts → Add a State. This is the preferred method.

  2. Through the request form — use this form if you don't have platform access, or if you're an Agency Principal submitting on behalf of downline agents.

Processing takes up to 7 business days after submission.

✅ After submitting, your license is automatically sent to the carriers you selected. Once each carrier processes the appointment, it will appear in your Spark platform. If it hasn't shown up after 7 business days, email [email protected] with the carrier name and state(s).

Carrier-specific notes

  • UnitedHealthcare and Anthem / Elevance: State appointment fees apply ($50–$200 per year range). These are deducted from your next statement by the carrier directly.

  • Anthem / Elevance: Requires a new contract for each state you add. After submitting, you'll receive an email from Anthem within 3 days with a link to complete. Anthem is only available in select states — you'll see the current list in the form.

  • Medicare Supplement carriers: You do not need to submit a state license in advance. Nearly all Med Supp carriers use just-in-time appointments.

Urgent requests

If you need to sell a plan with an effective date within 3 days, select Yes on the urgency question in the form. Note that Spark cannot guarantee the appointment will be accepted in time — the team will mark it as urgent and escalate to the carrier, but processing timelines are outside of Spark's control.
